Abstract

ABSTRAK Keselamatan pasien merupakan aspek mendasar dalam pelayanan kesehatan yang didefinisikan sebagai upaya pencegahan terjadinya cedera maupun risiko selama proses perawatan. Sebanyak 80% kejadian jatuh dipicu oleh miskomunikasi, terutama saat serah terima informasi antar perawat dan dokter. Penelitian ini bertujuan untuk menganalisis pengaruh implementasi komunikasi SBAR dalam hand over terhadap risiko jatuh pada pasien di Instalasi Rawat Inap A RSPAD Gatot Soebroto. Penelitian ini menggunakan desain deskriptif korelatif dengan pendekatan cross-sectional. Sampel penelitian terdiri dari 64 perawat di RSPAD Gatot Soebroto, menggunakan metode purposive sampling. Data dikumpulkan menggunakan kuesioner demografi, kuesioner implementasi komunikasi SBAR, dan kuesioner penerapan risiko jatuh. Hasil analisis univariat menunjukkan bahwa sebagian besar perawat berjenis kelamin perempuan, yaitu sebanyak 52 responden (81,3%). Mayoritas responden berada pada rentang usia 20-35 tahun sebanyak 37 responden (57,8%), serta memiliki latar belakang pendidikan D3 Keperawatan sebanyak 33 responden (51,6%). Implementasi komunikasi SBAR pada perawat menunjukkan bahwa sebagian besar 31 responden (48,4%) berada dalam kategori penerapan tinggi, penerapan upaya pencegahan risiko jatuh pada pasien menunjukkan bahwa sebagian besar 44 responden (68,8%) berada pada kategori penerapan tinggi. Selanjutnya, hasil analisis bivariat menggunakan uji Spearman Rank diperoleh nilai p = 0,008 dan Rho = 0,326. Hasil ini menunjukkan bahwa terdapat hubungan yang signifikan antara implementasi komunikasi SBAR dan penerapan upaya pencegahan risiko jatuh pada pasien. Rumah sakit diharapkan meningkatkan konsistensi penerapan komunikasi SBAR pada seluruh perawat saat proses hand over, baik pada pergantian shift, komunikasi antar profesi, maupun pemindahan pasien antar-unit, guna memperkuat upaya pencegahan risiko jatuh. As many as 80% of falls are caused by miscommunication, especially during the handover of information between nurses and doctors. This study aims to analyze the effect of implementing SBAR communication in handovers on the risk of falls among patients in the Inpatient Ward A at Gatot Soebroto Army Hospital. This study uses a correlative descriptive design with a cross-sectional approach. The study sample consisted of 64 nurses at Gatot Soebroto Army Hospital, using purposive sampling. Data were collected using demographic questionnaires, SBAR communication implementation questionnaires, and fall risk application questionnaires. The univariate analysis results showed that most nurses were female, namely 54 respondents (81.8%). The majority of respondents were in the 20-35 age range, namely 39 respondents (59.1%), and had a D3 Nursing education background, namely 34 respondents (51.5%). The implementation of SBAR communication among nurses showed that most of the respondents, namely 31 respondents (47.0%), were in the high implementation category. The implementation of fall risk prevention efforts in patients showed that most of the respondents, namely 44 respondents (66.7%), were in the high implementation category. Furthermore, the results of bivariate analysis using the Spearman Rank test obtained a p-value of 0.003 and Rho = 0.366. These results indicate that there is a significant relationship between the implementation of SBAR communication and the implementation of fall risk prevention efforts in patients. Hospitals are expected to improve the consistency of SBAR communication implementation among all nurses during the handover process, whether during shift changes, interprofessional communication, or patient transfers between units, in order to strengthen fall risk prevention efforts.
